Please Fill in the form below and click submit if you would like Macmillan Caring Locally (MCL) to claim Gift Aid on any donations you have made in the past four years and any future donations you make.
By agreeing to this declaration you are confirming that you have paid or will pay an amount of income tax and / or capital gains tax for the current tax year (6 April to 5 April) that is at least equal to the amount of tax that all the charities that you donate to will reclaim on your gifts for the current tax year.
Other taxes such as VAT and council tax do not qualify. MCL will reclaim 25p of tax on every £1 that you have given.
You agree to notify MCL if you wish to cancel this declaration, if you change your name or address, or if you no longer pay sufficient tax on your income and / or capital gains.
